Hysteria is a neurotic overreaction to an experience or situation. You are not hysterical in the medical sense of the word if you normally react to moments of stress by crying or screaming uncontrollably. Many people tend to over-dramatize their feelings and are not mentally ill as a result. The illness known as hysteria, sometimes called a conversion reaction, occurs when someone (who is not normally "hyper") reacts to severe stress with physical symptoms that cannot be attributed to physical factors. These people are unaware that their symptoms are caused by hysteria. They, and usually their families and friends, simply assume that they are affected by a real physical disorder. Often, it is a problem that helps the hysterical person escape a stressful situation. For example, if you witness a terrible accident at your workplace, you may develop weakness in your legs that prevents you from leaving the house the next day or even for an extended period of time. Or total memory loss (amnesia) may follow an accident that you would prefer to forget.
Do not confuse hysteria with psychosomatic illness. Psychosomatic illnesses are influenced by stress, but they are actual physical disorders. Hysteria is a purely imaginary illness.
What are the symptoms?
The hysterical reaction may be relatively mild (e.g., vague pain, weakness, or dizziness) or extremely severe (paralysis of the limbs or sudden blindness). Less commonly, memory loss may also occur. Symptoms of illness associated with hysteria are difficult to diagnose, especially since the ill person denies that it is an emotional problem.
What should be done?
If you suspect that the disability of someone in your family is due to a neurotic reaction to an experience or situation, you should consult your doctor. Whether or not your suspicions are correct, you should never accuse someone of faking symptoms. Hysterical people are sick and often hypersensitive. Because hysteria is extremely difficult to diagnose, your doctor will probably want to order inpatient tests to rule out a physical cause for the symptoms. If the cause appears to be hysteria, the person will likely be referred to a specialist.
What is the treatment?
The goal of treatment is to find out the underlying problem and help the person resolve it. No medical treatment can cure the symptoms of hysteria, so it all depends on compassionate, patient psychotherapy. The only medication that can be prescribed for hysteria is a sedative that helps the person relax while the underlying problem is uncovered and the symptoms are gradually eliminated. In rare cases, a treatment known as "abreaction" may be recommended, which includes medication. It is best suited for people whose hysteria is due to a single, severe emotional shock.
A person undergoing abreaction must lie down in a quiet, darkened room. To overcome resistance to talking about a painful subject, the person is put into a hypnotic state by either breathing in ether or being injected with a special drug. Then, when the person is completely relaxed, he or she is asked to describe the incident that triggered the hysterical reaction in a detailed illness treatment. Simply reliving a repressed experience in this way often eliminates the symptoms of hysteria. No one knows why this happens, although there are a number of interesting theories.
